Sports Physicals
We offer quick and comprehensive sports physical evaluation (also known as Pre-participation physical evaluation / Camp Physical / School Physical). Most schools and sport teams require a physician evaluation of fitness of children to participate in activities. A Sports Physical includes the following:
- Basic vitals are recorded (height, weight, BP and heart rate)
- A thorough review of the patient’s medical and family history including prior symptoms with exercise
- General physical exam including scoliosis exam
- Completion of the forms to provide necessary information
A sports physicals DOES NOT replace your child’s Comprehensive Annual Well Check!
- If your child requires a Sports Physicals, call us at 972-893-1192 for an appointment. Walk-ins are also welcome! Sports physicals cost $40
- Please remember to bring the form provided by the school or team that needs to be completed and signed by the physician
- If you are not currently a patient of Care & Cure Pediatrics, please also bring with you a copy of your child’s most recent immunization record, any recent physical exam document and valid identification document(s) recognized by the State of Texas
