A Sports Physical Is Not a Well Check
A sports physical does not replace your child’s annual well-check visit — they serve different purposes. The sports physical focuses on clearance for physical activity, while the well check is a comprehensive preventive health visit. Insurance covers well checks; sports physicals are typically self-pay at $50.

Bring the physical form from your school, league, or camp. We complete and sign it on the spot.
Injury History
If your child has had prior injuries or surgeries, or wears orthotics or braces, let us know in advance.
Medications
Bring a list of all current medications, including supplements and any inhalers used during exercise.
If Dr. Vani identifies a concern (a heart murmur, high blood pressure, or a musculoskeletal issue), she may recommend further evaluation before clearance is given. This is for your child’s safety — most concerns are resolved quickly with appropriate follow-up.
Simple, Flat $50 Fee
Sports physicals are $50, self-pay only. They are typically not covered by insurance, since insurance covers annual well checks (a separate visit). We accept cash, debit, and credit cards. A 3% convenience fee applies to card payments.
Frequently Asked Questions
Can I walk in for a sports physical?
Yes — walk-ins are welcome. Calling ahead is always helpful so we can estimate wait times and ensure a provider is available. During back-to-school season (July–August) we see high volume, so calling ahead is especially recommended then.
Is the sports physical covered by insurance?
Sports physicals are generally not covered by insurance and are billed as self-pay at $50. Your annual well check is a separate, insurance-covered preventive visit. We recommend scheduling both — a well check for comprehensive preventive care, and a sports physical when your school or activity requires the specific form.
My child has asthma — will they be cleared?
Most children with well-controlled asthma are cleared for full sports participation. Dr. Vani will review your child’s asthma management, ensure they have a rescue inhaler available, and may complete an exercise-induced asthma section on the form if needed. Bring your child’s current asthma medications to the appointment.
How long is the sports physical valid?
Most schools and organizations accept a sports physical for one year from the date of the exam. Check your specific school or league’s requirements. If your child’s physical expires mid-season, we can perform an updated exam.
What happens if my child is not cleared?
If Dr. Vani identifies a potential concern, she will explain the finding and recommend appropriate next steps — which might include an EKG, echocardiogram, orthopedic evaluation, or specialist consult. Most children are ultimately cleared once the concern is investigated. Your child’s safety is the priority.
