Dr. Vani Venkatachalam is a designated US Civil Surgeon who is authorized to perform medical exam required for US permanent resident (green card applicants) of all ages (kids and adults). Please note that this medical exam does not replace the regular full medical examination, or diagnosis or treatment provided by your primary care provider. Typically 2 visits are required to complete the process:
Visit 1: For the visit, applicants must bring with them the following:
- Valid government issued photo ID card (e.g. passport, drivers license)
- Form I-693, Report of Medical Examination
- Available immunization records, including from childhood if available
- List of medications for treatment of any chronic conditions
- Report of the condition and any special education or supervision requirements (if anyone in your family is immigrating with learning disabilities).
We will verify and review information and complete medical exam. We will order USCIS-required laboratory tests and advice / provide on vaccine requirements. Once we have received all records and test results from the lab, we will call you for Visit 2.
Visit 2 The completed and sealed I-693 form will be given to you to submit to USCIS.
Service Fee: Immigration medical exam is a self-pay service. We will not be able to assist with insurance filings. The fee for medical exam and labs (vaccines not included) is $475 for applicants 15 years and older. Fee for applicants under 14 years is $375. The cost of an additional I693 form is $75. Full payment is due at the first visit. We accept cash, debit cards or credit cards. We do not accept checks. A convenience fee of 3.00% will be charged for all card transactions.
